Dosage Guidelines

These guidelines are intended as a general framework. Individual responses to cannabis can vary significantly, so personalisation of dosage is key.

- THC FLOWER -

Starting Dosage (Low-Tolerance or New Patients)

Inhalation (Vaporising): Start with 1-2 small puffs, wait 15-30 minutes, and assess the effects before considering more. Gradually increase if necessary.

Moderate Dosage (Experienced Patients)

Inhalation (Vaporising): 3-5 puffs, with careful monitoring of effects, especially for those with moderate tolerance.

High Dosage (High-Tolerance or Severe Symptoms)

Inhalation (Vaporising): More than 5 puffs may be appropriate for patients with high tolerance, but they should still monitor their response closely.

Titration Instructions

Consistent with TGA recommendations and international guidelines, the general approach to dosing should be to 'start low, go slow'. Patients should be advised that a titration period of several weeks may be required to determine the optimal dose.

0.1g > 0.2 g> 0.3 g> 0.4 g> 0.5g 0.2g > 0.4g > 0.6g > 0.8g > 1g - Increase until desired results are met.

Special Considerations

Elderly Patients: Start with the lowest possible dose, especially with edibles or tinctures, due to potential sensitivity to THC and slower metabolism.

Combination with CBD: Consider recommending a balanced THC ratio to help mitigate potential adverse effects like anxiety or paranoia.

Dosing Frequency

Inhalation: Effects typically last 2-4 hours, so patients may need to dose 2-3 times per day depending on their condition.

- CBD OILS -

When advising on the dosage of CBD oil, doctors should consider the patient’s specific medical condition, overall health, and experience with CBD.

Starting Dosage (New Patients or Those Sensitive to CBD)

Starting Dose: Begin with 5-10 mg of CBD per day. This could be 0.5 ml to 1 ml of the 100mg CBD oil, depending on the concentration of the oil (e.g., 10mg of CBD per ml).

Administration: Recommend taking the dose once daily, preferably in the evening to monitor how the patient responds.

Titration: After a few days, if well-tolerated and more relief is needed, the dose can be gradually increased by 5 mg increments every 3-7 days.

Moderate Dosage (Patients with Mild to Moderate Symptoms)

Dose Range: 10-20 mg of CBD per day (1-2 ml).

Administration: This dose can be split into two administrations (morning and evening) if needed.

Titration: Gradually adjust the dosage based on the patient’s response, ensuring they reach their desired level of symptom relief without experiencing significant side effects.

High Dosage (High-Tolerance or Severe Symptoms)

Dose Range: 20-30 mg of CBD per day, although doses higher than this are typically for more potent CBD oils.

Administration: Can be divided into two or three doses throughout the day, depending on the patient’s condition and how they metabolize CBD.

Titration: Increase gradually if needed, while monitoring for any side effects or drug interactions, especially in patients with complex medical histories.

Special Considerations

Elderly Patients: Start with the lowest possible dose (5 mg per day) and increase very slowly to account for slower metabolism and potential interactions with other medications.

Patients on Other Medications: Careful monitoring and possibly lower starting doses are recommended, as CBD can interact with certain medications, particularly those metabolized by the liver.

Monitoring for Side Effects: Regular follow-up is important, particularly when adjusting doses, to ensure efficacy and safety.

Dosing Frequency

Single Dose: For general wellness or mild symptoms, a single daily dose in the evening may be sufficient.

Multiple Doses: For chronic conditions, splitting the total daily dose into two or three administrations can provide more consistent relief throughout the day.

General Side Effects

Here’s a list of general side effects that might be experienced by patients:

Side Effects

Dry Mouth: A feeling of dryness in the mouth, which can usually be alleviated by staying hydrated.

Dry Eyes: A sensation of dryness or irritation in the eyes, which can be uncomfortable for some patients.

Increased Heart Rate: THC can cause a temporary increase in heart rate, which may be concerning for patients with cardiovascular issues.

Dizziness or Lightheadedness: Especially upon standing, due to THC's effects on blood pressure.

Impaired Memory and Cognition: Short-term memory and cognitive function may be temporarily affected, which can impair decision-making and focus.

Impaired Coordination: THC can affect motor skills and coordination, increasing the risk of accidents, especially when driving or operating machinery.

Paranoia or Anxiety: Some patients, particularly those new to high-THC strains, may experience increased anxiety or paranoia.

Precautions

Mental Health Disorders: Patients with a history of psychosis, bipolar disorder, or severe anxiety should not be prescribed medicinal cannabis, as high-THC strains may exacerbate these conditions.

Cardiovascular Conditions: Due to the potential for increased heart rate and blood pressure, patients with heart disease should use THC cautiously.

Pregnancy and Breastfeeding: THC is not recommended during pregnancy or breastfeeding due to potential risks to the fetus or infant.

Managing Side Effects

Start Low and Go Slow: Especially for new patients or those with lower tolerance, starting with a low dose can help mitigate side effects.

Stay Hydrated: Encourage patients to drink plenty of water to manage dry mouth and dizziness.
